Electronic Disclosure and Consent Agreement
This Electronic Disclosure and Consent Agreement (the "Agreement”) contains important information about how we may deliver disclosures required by applicable law. This includes periodic deposit account statements (E-Statements), annual error resolution notices, change-in-terms notices, and our annual privacy notice. Your consent to receive disclosures, whether available now or in the future, applies to any and all disclosures that we elect to provide to you electronically at our discretion. You further acknowledge that by accessing the Bank's website at www.fnbga.com, you satisfy the hardware and software requirements discussed more fully below and accordingly acknowledge that you reasonably demonstrate that you can access, receive and retain disclosures electronically in the formats used, regardless of whether the disclosure is posted on the Bank's website, sent to you by e-mail or other electronic means.
This is your copy of the Agreement. Please read it carefully, print or download and retain a copy of it for your records. At the end of the Agreement, we will ask you to accept disclosures in an electronic format, rather than a paper format. If you agree, click the “I Agree” button at the end of the Agreement.
ENROLLING FOR E-STATEMENTS
You may enroll for E-Statements at any time via the Bank’s website www.fnbga.com.
You have the option to receive E-Statements for any or all of your First National Bank of Georgia online accounts.
There is no charge for E-Statements.
ACCOUNTS WITH MULTIPLE OWNERS
For accounts with multiple owners, only one account owner need enroll an account(s) in the service. In accordance with the terms of the applicable account agreement and disclosure statement, all account owners will be bound by the decision of the account holder who enrolls in or cancels the service. If any account, which you have applied for and have been approved by the Bank to receive E-Statements on, is a joint account, please be advised that only you will receive and be able to access the E-Statement for such account. Therefore, you are solely responsible for promptly and timely sharing any account statement or any statement related information provided thereon with your joint account holder as the need may be and/or as requested of you by any respective joint account holder.
CANCELING E-STATEMENTS
You may opt-out of E-Statements at any time by logging in to the Bank’s online banking service, selecting “View Statements”, clicking on “Click here to complete the request form” and completing the request form. If you opt-out of E-Statements, we will resume delivery of your paper statements by U.S. Mail. There is no penalty associated with the reinstatement of paper statements.
E-STATEMENT DELIVERY
By enrolling in E-statements, you have convenience and security, and you can eliminate the delivery of paper statements.
Our E-Statement product is easy to use, simply log in to First National Bank of Georgia’s online banking service and select “view statements”.
If you enroll for E-Statements, you may receive a paper statement for the first statement cycle in addition to the electronic version. After the first statement cycle, your periodic account statements will only be accessible through the Bank’s online banking service.
EMAIL ADDRESS CHANGES
If you enroll for E-Statements, you will receive an email alert at the email address you provide to us when your E-Statement is available. You can change the email address for the statement alert at any time by clicking on “my profile” then select “change profile information” within the Bank’s online banking service, or by visiting any First National Bank of Georgia location.
It is your sole responsibility to provide us with your correct contact information, including your email address.
E-STATEMENT AVAILABILITY
E-Statements may be viewed, printed and downloaded for permanent retention from the Bank’s online banking service for 24 months from the date the
E-Statement is made available.
You may also save copies of your E-Statements to your hard drive or other media for viewing or printing at a later time.
In order to view, print, download and save copies of your E-Statements, you must have the hardware and software described below.
If you need a paper copy of your statement, please contact any First National Bank of Georgia location.

An External Email Address
A computer and Internet browser that can support 128-bit encryption (For security purposes, you should use the most current version of either: Microsoft Internet Explorer, Mozilla, FireFox or Netscape for Windows and Safari 2.0.1 for Mac. The most current browser versions are typically more secure and will support 128-bit encryption.)
You must have Adobe Reader 7.0 or above installed on your PC in order to view your E-Statements. If you do not already have Adobe Reader, you may access a free download at www.adobe.com by selecting the Free Adobe Reader under the Download section.
Access to a printer or storage medium such as a hard drive so that you can download and/or print disclosures and/or statements for your records
We may revise hardware and software requirements. If there is a material chance that the changes may impact your ability to access E-Statements, we will notify you of these changes in advance.
PROCEED WITH ACCEPTANCE OF ELECTRONIC DISCLOSURES
With your acceptance below, you agree to accept delivery of disclosures in an electronic format. You also agree that you have the necessary equipment for accessing and viewing electronic disclosures and you agree to notify us if you change your email address or if you no longer want to receive disclosures electronically.
If you decide not to enroll for E-Statements, select the “I DISAGREE” button below. You will have the option to enroll at any time via the Bank’s website www.fnbga.com. If you click on “I AGREE” below, you will be directed to an Enrollment form. Please read and complete all required fields. After completing the enrollment form, click “SUBMIT” and your information will be forwarded to us for processing.
